Customer Service

Review billing statements and contact Customer Service if you believe that your statement is not correct, need an explanation of the statement or can provide us with updated information.

Contact Customer Service if you cannot pay your balance in full. For faster service, please give Customer Service your account number.

Your first statement will be sent to you within 30 days after the date your service provided. The statement will identify what we have billed to insurance and what is your responsibility to pay. For your convenience we accept most major credit cards, personal checks or cash. Payment can be made at your doctor’s office or sent to the address below. Credit card payments may also be made by contacting Customer Service at (630) 942-7998.

When you contact Customer Service please have your account number ready. This will help Customer Service access your account more quickly. Your account number is located in the upper left hand corner of your billing statement.

Referral Management

Referrals for medical services must be initiated by a DuPage Medical Group physician. If you are a DuPage Medical Group patient and you need a referral for services, please contact your Primary Care Physician. Once a referral has been generated by your physician, the Referral Management staff will work with your insurance company to obtain the necessary authorization for services. The length of the authorization process varies by health plan and service. Please call the Utilization Management Department at 630-942-7995 if you have any questions relating to a pending referral. The Utilization Management staff is available to receive your calls Monday through Friday from 8:00 am to 4:30pm.

Registration

Patient Registration
Below you will find downloadable patient registration forms that can be printed and competed prior to your office visit. Completing these forms before your visit will reduce your registration time.

New Patients
Please complete a Patient Registration Form before your appointment. Plan on arriving 15 minutes prior to your scheduled visit time to finalize your registration. Please be sure to bring the completed registration form and a copy of your insurance cards.

Existing Patients
If any patient information has changed since your last visit, for example, your insurance, your address or employer, please complete a Patient Registration Form as well. Plan on arriving 15 minutes prior to your scheduled visit to finalize your registration. Please be sure to bring the completed registration form and a copy of your insurance cards.
