This status assures you that we offer high-quality educational services and meet the National Standards for Diabetes Self-Management Educational Programs.
Our program includes 1-1 consults or group education sessions supervised by our Certified Diabetes Educators at 5 convenient locations with state-of –the art diabetes care. Each location is staffed with Certified Diabetes Nurse Educators and Dietitians who offer the following services:
Type 2 group education – a three series class geared toward your understanding of diabetes management including: glucose monitoring, nutrition, exercise, medications and personalized goal setting.
Medical Nutrition Treatment (MNT)-develops a meal plan for you
One- One consults personalized instruction
