to care for you in the same warm and friendly manner
we would treat our own family members. We believe
in utilizing open communication, education and
preventative health to ensure your wellness. You are
encouraged to take an active role in your healthcare
and we welcome any questions about your health.

Appointments
Appointments may be scheduled during regular office
hours at 630 241 1000.
Please inform the receptionist of the nature
of your visit so adequate time is allowed for your
appointment. To cancel an appointment, please
contact us 24 hours prior to your scheduled visit.
Patient Registration
New patients are asked to come early to their first
appointment. Please bring the following with you:
- A list of medications and over-the-counter
drugs you are currently taking

Insurance verification card
Referrals to Specialists
To check on the status of a referral, contact
the referral coordinator at 630 241 1000.
Emergency Calls
Illness is rarely convenient and emergencies occur at
all times. In the event of an emergency or serious illness
after office hours, our answering service will contact
us immediately to return your call. If an emergency
occurs during regular office hours, inform our staff so
we can contact you immediately. If you need immediate
medical care, please call 911 or proceed to the nearest
emergency room. The emergency room staff will contact
us regarding your care. If you have HMO insurance,
you are required to obtain pre-authorization for
emergency department visits from your physician,
except in the case of life-threatening emergencies.
Hospital Affiliations
Our physicians are on staff at Edward Hospital in
Naperville and Good Samaritan Hospital in Downers
Grove. Both are state-of-the-art healthcare facilities.
Test Results
Generally, you should plan to call our office for
your test results several days after they are taken.
It is our practice to notify parents of significant test
results as soon as possible. Because we may need
to reach you if results require immediate attention,
please be certain to supply us with your current
home and work telephone numbers.
Medication Refills
Please allow 48-72 hours for a response to your
request for a medication refill. Narcotic and controlled
substances refills are done by a patient’s primary
physician and are not done on weekends or after
hours. To avoid any lapse in your medication regimen,
we recommend that patients discuss the need for
medication refills with the nurse or physician during
their scheduled doctor’s appointment.
Medical Confidentiality
Your medical records are held in strictest confidence
and cannot be released to anyone without your
written permission.
Release of Medical Records
A signed statement is required for the release of medical
records. Please allow three weeks to copy the records.
A minimal charge may be assessed for this service.
Young Visitors
Children are welcome at Woodridge Family Medicine.
For their safety and well-being, children should not be
left unattended anywhere in our office. Children under
the age of 18 who are scheduled for an appointment
and not accompanied by an adult must have a signed
letter of authorization for treatment and a phone number
at which parents may be contacted if we need to speak
to them directly. Parents are encouraged to provide
their child’s caregiver/sitter with a signed letter of
authorization for emergency treatment.
