Illness After Hours
A physician is on call after hours to answer your urgent medical questions. Call 630 435 9888 to contact the on call physician. If you have Managed Care insurance, you are required to obtain pre-authorization for Emergency Department visits from your physician, except in the case of life-threatening emergencies.

All x-rays require a physician’s order. No appointments are necessary.
Referrals
To request a referral, please contact our referral coordinators at 630 435 9888. To check on the status of a pending referral, contact Utilization Management Department at 630 942 7995.
Reminders
As a courtesy, we will call to remind patients of upcoming appointments. Patients will receive this notification 48 hours prior to their scheduled appointment.
Cancellations
Please provide at least 24 hours notice for any cancellation. Patients can cancel an appointment by calling the office at 630 435 9888.
Medication Refills
All medication refills are done during regular office hours only. Please allow 72 hours for a response to your refill request. Refills should be initiated by contacting your pharmacy directly. Narcotic and controlled substance refills are done by a patient's primary physician and are not done on weekends or after hours. To avoid any lapse in your medication regimen, we recommend that patients discuss the need for medication refills with the nurse or physician during their scheduled doctor’s appointment.
