    ANNUAL APPOINTMENTS

    Please call the office at least one month prior to your needed appointment date. Please alert our schedulers and your physician that you have preventative care insurance coverage during appointment scheduling.

    CONCERNS REGARDING YOUR RECENT TEST RESULTS

    Please schedule an appointment with your physician to discuss your test results.


    Reminders

    As a courtesy, you will receive a phone call reminder 48 hours before your appointment.


    Cancellations

    We understand that things come up that will cause you to cancel your appointment. We would appreciate at least 48 hours notice. We make it a point to respect your time and make every effort to be on time for your scheduled appointment.


    Illness After Hours

    One of our physician’s is on call 24 hours, 7 days a week to manage your emergent needs. Call 630 323 3540 to contact the on call physician.

    All billing issues are directed to our customer service department who will put you in contact with your account representative to research your billing issues.


    Check-in/Registration

    Please bring your insurance information and photo identification to all appointments.


    Medication Refills

    Please bring a list of your current medications with you to your scheduled appointments so we can update your medical records. Medications are refilled once a year at your annual appointment.

    Maintenance medications are refilled on weekdays during normal business hours, but not on weekends.

    Narcotic medications are only refilled during normal business hours.

    LOCAL PHARMACY

    Please call your pharmacy at least 3-5 business days before you will run out of medication and have your pharmacy fax us your refill requests at (630) 323-9079.

    Please call your pharmacy to see if your prescription is ready before picking it up as it can take 2 business days to complete your order.

    MAIL-IN PHARMACY

    Please contact us at 2 weeks before you anticipate running out of your medication. We can assist you with completing your pharmacy forms. In addition, we can mail your prescription or arrange for you to pick it up.

    Please be advised that we can no longer fax in your mail order prescriptions to your pharmacy provider.


    Test Results

    You will be notified of your test results by mail or phone call within 7-10 business days. Please understand, test result calls cannot be returned on the weekends.

    If you have not heard from our office regarding your test results within this timeframe, please call our office.


    Messages

    All phone messages are relayed to your physician’s nurse. Depending on your issue, we will try to return calls on the same day. However, at times, returned calls may be made the next day. Be sure to leave phone numbers and times we can use to return your call in a timely manner.

    If you are not feeling well or are concerned about symptoms, we ask that you schedule an appointment with your physician to assess your symptoms.


    Form Completion

    Dictated letters, disability forms, parking permits and copies of chart records typically require 1-2 weeks for processing and fees may apply. Comprehensive forms may require an in-office visit to complete.


    Referrals

    REFERRALS TO SPECIALISTS OR TEST CENTERS WITHIN DUPAGE MEDICAL GROUP

    Wait 3 business days to schedule your appointment within DuPage Medical Group. Our referral coordinator will enter your authorization numbers in the DuPage Medical Group computer system.

    Your insurance will dictate the need for a referral to a specialist or a diagnostic test. If you are not sure if you will need a referral, please contact your insurance company for directions.

    Medicare patients do not need referrals. However, if you have a supplemental or secondary managed care plan to Medicare, they may require a referral.To contact the Utilization Management Department, please call 630 942 7995.

    REFERRALS TO SPECIALISTS OR TEST CENTERS OUTSIDE OF DUPAGE MEDICAL GROUP

    Our Referral Coordinator will contact your insurance company for you to obtain an authorization for your appointment. This referral with the authorization number will be mailed to your home within 5-7 business days after receiving approval from your insurance company. Please wait to schedule your appointment until you receive this referral.
