Preparing for Your Appointment

Prior to your first appointment, please complete the Genetics Questionaire and Personal History Forms and bring them to your first appointment. Also bring your insurance information to your first appointment. This is very important, as it will keep you from receiving unneccessary bills related to your care.

At Your First Appointment

During your first appointment, you will receive your first prenatal check-up. Your doctor and nurse will discuss your pregnancy with you and answer any questions you may have. You will also receive a booklet. This publication is an excellent resource for you. It contains great information about pregnancy, delivery and post delivery. It also contains information about your hospital. We hope you find it useful and enjoyable.

Illness After Hours

A physician is on call after hours to answer your urgent medical questions. If you have HMO insurance, you are required to obtain pre-authorization for emergency department visits from your physician, except in the case of life-threatening emergencies.

Referrals to Specialists

Referrals to specialists within DuPage Medical Group usually are processed in 1-3 business days and referrals to specialists outside of DuPage Medical Group require 3-5 business days for processing. To check on the status of a referral, contact utilization management at 630 942 7995.

Medication Refills

Please allow 48-72 hours for a response to your request for a medication refill. Narcotic and controlled substances refills are done by a patient’s primary physician and are not done on weekends or after hours.
