- A copy of your XRAYS’s/MRI’s/CT scans (not the reports - get the actual films)
- Copies of records from doctors related to your visit with us
- Spine Center Patient Information Form
- Patient registration Sheet
- HIPAA acceptance
- Release of Confidential Information
