At DuPage Medical Group we focus on the patient as a whole, with access to not only physicians and specialists, physical therapists, dieticians, and lab or diagnostic imaging facilities. For convenience, our Physiatry department is at multiple locations across DuPage County. We can treat adults of all ages, as well as adolescents.
To prepare for your appointment
To make our appointment with you as beneficial as we can, please bring with you the following:
- Health Insurance Card/Information
- It may be helpful to bring friend or family member to take notes/help with questions
- A list of all medications you are taking
- A list of your questions /concerns
- A copy (and if possible, the written report) of any x-rays, MRI or other diagnostic tests that you may have completed at a facility other than DuPage Medical Group
- Copies of records from doctors related to your visit with us
