How to Request Medical Records
To obtain copies of your medical record, please complete an Authorization for Release of Health Information Form.
To have a copy of your medical records sent to DuPage Medical Group from another facility, please contact that provider directly.
DMG is unable to release records for Edward-Elmhurst Healthcare. Please contact them directly for your records from those locations.

There are no fees associated with requests for information to be sent directly to another medical provider outside of DMG. There may be fees associated for requests to be sent to you or to someone who is not a medical provider. If there is a fee for your request, RecordConnect will provide you with an invoice summarizing the charges. Payments can be made over the phone by calling (312) 757‑5020 or by mailing a check to the address listed on the invoice. Accepted forms of payment are check or credit card. Cash payments are not accepted. All fees are in accordance with Illinois and federal law.
Records requested by the patient can be sent securely via e‑mail, picked up at 801 Ogden Avenue in Lisle, IL, or mailed to an address specified. Please indicate your preferred delivery method when making your request. Please allow 7 to 14 business days for delivery from the date of the request or the date payment is received if there was a charge.
