Medical Records Requests

DELIV­ERY METHOD UPDATE — Coro­n­avirus (COVID-19): To pro­tect you, our patients and our staff, we rec­om­mend request­ing your med­ical records be sent secure­ly via e‑mail or mailed to a pre­ferred address instead of in-per­son pick-up for paper copies.

How to Request Med­ical Records

To obtain copies of your med­ical record, please com­plete an Autho­riza­tion for Release of Health Infor­ma­tion Form.

To have a copy of your med­ical records sent to DuPage Med­ical Group from anoth­er facil­i­ty, please con­tact that provider directly.

DMG is unable to release records for Edward-Elmhurst Health­care. Please con­tact them direct­ly for your records from those locations.

Infor­ma­tion you are request­ing may be avail­able through MyD­MG Health: https://​mychart​.dupagemed​ical​group​.com.

Fees

There are no fees asso­ci­at­ed with requests for infor­ma­tion to be sent direct­ly to anoth­er med­ical provider out­side of DMG. There may be fees asso­ci­at­ed for requests to be sent to you or to some­one who is not a med­ical provider. If there is a fee for your request, Record­Con­nect will pro­vide you with an invoice sum­ma­riz­ing the charges. Pay­ments can be made over the phone by call­ing (312) 7575020 or by mail­ing a check to the address list­ed on the invoice. Accept­ed forms of pay­ment are check or cred­it card. Cash pay­ments are not accept­ed. All fees are in accor­dance with Illi­nois and fed­er­al law.

Deliv­ery

Records request­ed by the patient can be sent secure­ly via e‑mail, picked up at 801 Ogden Avenue in Lisle, IL, or mailed to an address spec­i­fied. Please indi­cate your pre­ferred deliv­ery method when mak­ing your request. Please allow 7 to 14 busi­ness days for deliv­ery from the date of the request or the date pay­ment is received if there was a charge.
