Getting vaccinated, along with safety measures like social distancing and masking, is critical in the fight to end the COVID-19 pandemic. All presently available vaccines offer substantial reduction in the risk of severe COVID illness, hospitalization and death.

As of Monday, April 12, everyone over the age of 16 is eligible to receive a COVID-19 vaccine in Illinois. We encourage everyone who is eligible to get a vaccine as soon as they are able to schedule one.

How to schedule your vaccine appointment

As of Monday, April 12, everyone over the age of 16 is eligible to receive a COVID-19 vaccine in Illinois. Appointments for first doses are made available as we receive inbound supply.

Complete the vaccine registration form and we'll email or text you a link to select an appointment. Links are sent within 60 minutes of registration based on supply and available appointments.

Vaccine Eligibility

Because there are not enough vaccines currently available for everyone in the country to be vaccinated, the Centers for Disease Control and Prevention as well as the Illinois Department of Public Health have provided recommendations on who should be offered the COVID-19 vaccine first. These recommendations are made with the intention of decreasing death and serious disease, preserving functioning of society and reducing the burden COVID-19 bears on people already facing disparities. DuPage Medical Group is working closely with the IDPH and local health departments and is following guidelines set forth for our state.
